| INSURANCE AND BILLING |
| Physical therapy is a
covered benefit under most insurance plans. Benefits, deductibles and co-payments vary
greatly with each plan and you are encouraged to call your insurance plan for details of
your individual benefits. We will verify your benefits prior to treatment however; this
is not a guarantee of payment. You are responsible for payment of services rendered to you
regardless of coverage status.
Currently OWEN Physical Therapy Inc. participates with: v Medicare v Blue cross blue shield v PPOM v Aetna v Tricare v McLaren Health Advantage v McLaren HMO This list is continually being updated so if your insurance is not mentioned please call us to see if we participate with them. For those without insurance, we also take private pay and accept cash or check Please bring all applicable insurance cards, forms and claim numbers necessary for insurance billing to your first appointment and let us know if you have any changes in insurance coverage, employer or home address. |
